I pazienti che presentano questioni riguardanti il genere ci costringono a un’opera di decentramento rispetto a pratiche e a teorie date per scontate. Due vignette cliniche di pazienti con questioni legate al genere sono qui ripensate a seguito della loro discussione all’interno di un gruppo di lavoro. Lo scopo è rendere conto di come, a partire dalla clinica, tali occasioni di dialogo abbiano permesso di dare voce alla pluralità di identificazioni su cui si costruisce l’identità. La paura della stigmatizzazione dei pazienti da parte della società, potrebbe presentarsi parallelamente nel terapeuta come paura di non essere considerato sufficientemente “ortodosso” dai suoi colleghi rispetto alle teorie di riferimento. In questo senso queste occasioni di dialogo aperto e non giudicante tra colleghi paiono di fondamentale importanza per accedere a un pensiero nuovo e creativo.

Rethinking gender together with gender-fluid patients. Two cases reconsidered starting from the group discussion: Patients who present gender issues force us to decentralize from practices and theories taken for granted. In this work, we propose two clinical vignettes of patients presenting gender issues and reconsider them starting from the discussion in working groups. The purpose of this work is to show how, starting from the clinical material, these opportunities for dialogue have given voice to the plurality of identifications on which identity is built. The patients’ fear of being stigmatized by society could present parallel the analysts’ fear of not being considered sufficiently “orthodox” towards reference theories, by the psychoanalytic societies. As we experienced writing this work, opportunities for an open and non-judgmental dialogue amongst colleagues seem to be fundamental for accessing new and creative thinking.

Repenser le genre avec les patients gender-fluid. Deux cas revisités par une discussion de groupe: Les patients qui présentent des problématiques liées au genre nous obligent à nous décentraliser des pratiques et théories tenues pour acquises. Deux cas cliniques de patients présentant des problématiques liées au genre sont revisités ici à la suite de leur discussion au sein d'un groupe de travail. L'objectif est d'illustrer comment, à partir du contexte clinique, ces opportunités de dialogue ont permis l'expression de la pluralité des identifications sur lesquelles se construit l'identité. La crainte des patients d'être stigmatisés par la société peut également se manifester chez le thérapeute en tant que crainte d'être considéré comme insuffisamment «orthodoxe» par ses collègues quant aux théories pertinentes. En ce sens, ces opportunités de dialogue ouvert et dénué de jugement entre collègues apparaissent fondamentales pour accéder à une réflexion nouvelle et créative.

Repensar el género junto a pacientes gender-fluid. Dos casos reinterpretados a partir de la discusión grupal:Los pacientes que presentan cuestiones relacionadas con el género nos obligan a un trabajo de descentramiento respecto a prácticas y teorías que se daban por sentadas. Dos viñetas clínicas de pacientes con cuestiones de género son aquí repensadas tras su discusión dentro de un grupo de trabajo. El objetivo es dar cuenta de cómo, partiendo de la clínica, estas ocasiones de diálogo han permitido dar voz a la pluralidad de identificaciones sobre las que se construye la identidad. El miedo a la estigmatización por parte de la sociedad puede presentarse paralelamente en el terapeuta como miedo a no ser considerado suficientemente “ortodoxo” por sus colegas respecto a las teorías de referencia. En este sentido, estas ocasiones de diálogo abierto y no juzgante entre colegas parecen de fundamental importancia para acceder a un pensamiento nuevo y creativo.

Mit genderfluiden Patienten Geschlecht gemeinsam neu denken. Relektüre zweier Fälle ausgehend von der Gruppendiskussion: Patienten, die Fragen bezüglich des Geschlechtes aufwerfen, zwingen uns zur Dezentrierungsarbeit in Bezug auf nicht hinterfragte Praktiken und Theorien. Zwei klinische Vignetten von Patienten mit Fragen in Zusammenhang mit dem Geschlecht werden hier im Anschluss an ihre Diskussion innerhalb einer Arbeitsgruppe neu gedacht. Das Ziel ist es, zu verdeutlichen, wie ausgehend von der Klinik solche Gelegenheiten des Dialogs es ermöglicht haben, der Pluralität der Identifizierungen, auf denen sich die Identität konstruiert, eine Stimme zu verleihen. Die Angst der Patienten vor der Stigmatisierung durch die Gesellschaft kann sich parallel im Therapeuten als Angst vor der Wahrnehmung durch Kollegen als nicht ausreichend ‚orthodox‘ in Bezug auf die Referenztheorien zeigen. In diesem Sinne scheinen solche Gelegenheiten des offenen, nicht verurteilenden Dialoges zwischen Kollegen von grundlegender Bedeutung zu sein, um zu neuem und kreativem Denken zu gelangen.

L'accostamento dei termini “violenzaˮ e “idealitàˮ invita a riflettere su una reciproca implicazione delle due sfere concettuali coinvolte. In termini psicoanalitici e metapsicologici la riflessione rimanda all'istanza del Super-Io in particolare ai processi di identificazione. Alcune sequenze cliniche tratte da un contesto penitenziario mostrano una particolare configurazione del legame paterno con l'emergere della fantasia “un padre viene picchiatoˮ in cui l'aspetto dell'ideale rischia di consolidare scenari caratterizzati da violenza. Le questioni dell'ideale e della violenza sono estese alla specificità del dispositivo clinico e della situazione psicoanalitica.

Violence and ideality: The juxtaposition of the terms “violence” and “ideality” invites us to reflect on a mutual implication of the two conceptual spheres involved. In psychoanalytic and metapsychological terms, reflection refers to the instance of the Superego, in particular to the processes of identification. Some clinical sequences taken from a penitentiary context show a particular configuration of the paternal bond with the emergence of the fantasy “a father is beaten” in which the aspect of the ideal risks consolidating scenarios characterized by violence. The questions of the ideal and violence are extended to the specificity of the clinical device and the psychoanalytic situation.

Violence et idéalité: La juxtaposition des termes “violence” et “idéalité” invite à réfléchir à l'implication mutuelle des deux sphères conceptuelles impliquées. En termes psychanalytiques et métapsychologiques, cette réflexion renvoie au Surmoi, et plus particulièrement aux processus d'identification. Certaines séquences cliniques issues d'un contexte carcéral révèlent une configuration particulière du lien paternel, avec l'émergence du fantasme «un père est battu», dans lequel la dimension idéale risque de consolider des scénarios caractérisés par la violence. Les questions de l'idéal et de la violence sont étendues à la spécificité du contexte clinique et de la situation psychanalytique.

Violencia e idealidad: La asociación de los términos “violencia” e “idealidad” invita a reflexionar sobre una implicación recíproca de las dos esferas conceptuales implicadas. En términos psicoanalíticos y metapsicológicos, la reflexión remite a la instancia del Superyó, en particular a los procesos de identificación. Algunas secuencias clínicas tomadas de un contexto penitenciario muestran una configuración particular del vínculo paterno con la aparición de la fantasía “un padre es golpeado”, en la que el aspecto del ideal corre el riesgo de consolidar escenarios caracterizados por la violencia. Las cuestiones del ideal y de la violencia se extienden a la especificidad del dispositivo clínico y de la situación psicoanalítica.

Gewalt und Idealität: Die Annäherung der Begriffe ‚Gewalt‘ und ‚Idealität‘ lädt zur Reflexion über eine wechselseitige Implikation der beiden beteiligten konzeptuellen Sphären ein. In psychoanalytischer und metapsychologischer Hinsicht führen die Überlegungen zurück zur Instanz des Über-Ichs, insbesondere zu den Prozessen der Identifizierung. Einige klinische Sequenzen aus dem Kontext des Strafvollzugs zeigen eine bestimmte Konfiguration der Vaterbeziehung mit dem Aufkommen der Phantasie ‚Ein Vater wird geschlagen‘, in der der Anblick des Ideals Gefahr läuft, von Gewalt gezeichnete Szenarien zu verfestigen. Die Fragen des Ideals und der Gewalt werden auf die Spezifizität des klinischen Dispositivs und der psychoanalytischen Situation ausgeweitet.

Tra interpretazione e sintonizzazione affettiva vi è sempre stato un particolare interesse da parte degli psicoanalisti, ma mentre l’interpretazione è stata la cifra della teoria e della clinica psicoanalitica, la sintonizzazione è sempre stata implicita e poco analizzata soprattutto dalla prima psicoanalisi. Freud stesso la lasciava al tatto e al tono dell’interpretazione. Sta di fatto che la sintonizzazione implicitamente comporta una visione relazionale e intersoggettiva della relazione analitica. In questa nota si propone che la sintonizzazione affettiva rappresenti il primo esito della tensione intersoggettiva tra analista e paziente e che si collochi in una dimensione più dell’ordine somatico che simbolico. Attraverso un esempio clinico si cerca di documentare come la sintonizzazione affettiva permetta di cogliere livelli di sofferenza altrimenti inaccessibili alla dimensione simbolica, dove la dissociazione, prima che la rimozione, rappresenta una sospensione del processo integrativo.

Between affect attunement and interpretation: Psychoanalysts have long demonstrated a distinct interest in the relationship between interpretation and affect attunement. While interpretation has traditionally been the cornerstone of psychoanalytic theory and practice, affect attunement, on the other hand, has largely remained implicit and underexplored, particularly in early psychoanalytic work. Freud himself left attunement to the nuances of the touch and tone of interpretation.

The fact is, affect attunement inherently entails a relational and intersubjective view of the analytic relationship. This paper argues that affect attunement represents the first outcome of the intersubjective tension between analyst and patient, operating within a somatic rather than symbolic domain. Through the use of a clinical vignette, we aim to demonstrate how affect attunement enables access to depths of suffering that would otherwise remain beyond the reach of the symbolic domain, where dissociation-occurring prior to repression-serves to suspend the integrative process.

Entre syntonisation affective et interprétation: Entre interprétation et syntonisation affective il y a toujours été un intérêt particulier par les psychanalystes. Cependant, si l'interprétation a toujours été la marque de la théorie et de la pratique psychanalytiques, la syntonisation affective a toujours été implicite et peu étudiée, surtout depuis les débuts de la psychanalyse. Freud lui-même l'a laissée à la sensibilité et au ton de l'interprétation. Il n'en demeure pas moins que la syntonisation implique implicitement une vision relationnelle et intersubjective de la relation analytique. Cet article propose que la syntonisation affective représente le premier résultat de la tension intersubjective entre l'analyste et le patient et qu'elle se situe dans une dimension plus somatique que symbolique. À travers un exemple clinique, nous tentons de démontrer comment la syntonisation affective permet d'appréhender des niveaux de souffrance autrement inaccessibles à la dimension symbolique, où la dissociation, avant que le refoulement, représente une suspension du processus intégratif.

Entre sintonización afectiva e interpretación: Siempre ha existido un interés particular entre los psicoanalistas por la relación entre interpretación y sintonización afectiva, pero mientras que la interpretación ha sido el signo distintivo de la teoría y la clínica psicoanalítica, la sintonización ha sido siempre implícita y poco analizada, especialmente en los albores del psicoanálisis. El propio Freud la dejaba al tacto y al tono de la interpretación. Lo cierto es que la sintonización implica una visión relacional e intersubjetiva de la relación analítica. En esta nota se propone que la sintonización afectiva representa el primer resultado de la tensión intersubjetiva entre analista y paciente que se sitúa en una dimensión más del orden somático que simbólico. A través de un ejemplo clínico se intenta documentar cómo la sintonización afectiva permite captar niveles de sufrimiento que de otro modo serían inaccesibles en la dimensión simbólica, donde la disociación, más que la represión, representa una suspensión del proceso de integración.

Zwischen Affektabstimmung und Deutung: Schon immer bestand seitens der Psychoanalytiker ein besonderes Interesse zwischen Deutung und Affektabstimmung, doch während die Deutung Chiffre der psychoanalytischen Theorie und Klinik war, war die Abstimmung immer implizit und wurde, insbesondere von der ersten Psychoanalyse, kaum untersucht. Freud selbst überließ sie dem Takt und Ton der Deutung. Tatsache ist, dass die Abstimmung implizit eine relationale und intersubjektive Auffassung der analytischen Beziehung mit sich bringt. Die vorliegende Notiz schlägt vor, dass die Affektabstimmung das erste Ergebnis der intersubjektiven Spannung zwischen Analytiker und Patient ist und sich eher in einer somatischen denn in einer symbolischen Dimension verortet. Anhand eines klinischen Beispiels wird versucht zu dokumentieren, wie die Affektabstimmung es erlaubt, der symbolischen Dimension ansonsten unzugängliche Ebenen des Leidens zu erfassen, wo die Dissoziation noch vor der Verdrängung eine Aufhebung des integrativen Prozesses darstellt.
